New Patient Bookings
To book your first appointment, please register as a “New Patient” via Health Engine, or call our friendly Reception Team on (08) 9751 4500 for assistance.
Once your appointment is booked, you’ll receive a link to complete our electronic New Patient Form. If you're unable to complete it online, please arrive 10 minutes early to fill out a paper copy at the clinic.

Cancellation Policy
If you are unable to attend your appointment, please cancel or reschedule at least two (2) hours in advance via the Health Engine app or by calling the clinic.
A $60.00 missed appointment fee will apply if sufficient notice is not provided. Early cancellation allows us to offer the appointment to another patient in need of care.
Transferring Medical Records from your previous Doctor
If you would like us to request your medical records from your previous practice, please speak to our friendly reception team.
By law, we require your written consent and signature to proceed with the request. Our team will provide you with the necessary Medical Records Request Form to complete and sign.
